Procedures for various cases of situations and Standard Processing Periods

Note: The processing period referred to in the table is an approximate schedule.

I have a dependent family member(s) Apply for the Dependent certificate. Issuance of Membership Card for dependent takes two weeks.
Report category change or confirmation of National Pension. (for the member’s dependent spouse) Report to us (PMAC) as to category change or confirmation of Category III National Pension Insured Person.
I lost or damaged my Membership Card or Membership Card for adependent Request Membership Card reissuance. Reissuance takes two weeks; however, the Membership Card will be reissued on the spot if you take the procedure at our headquarters or at mutual aid Administration division of Garden Palace.
I moved out to a new place Report the new address. Issuance of the Address Confirmation Notice takes two weeks; the Membership Card shall not be reissued.
Report the new address for the Accumulated Fund Mutual Aid Annuity. Address change report is due on the 25th of each month.
Report the new address for the Mutual Aid Term Insurance. Address change report is accepted at all times.
Report the new address for the National Pension (for the member’s dependent spouse). Address change notification should be submitted to PMAC.
One of my dependents got a job, income increased, or one of my dependents who is required to live with a member started living separately Request the deletion of dependent registration.  Make sure to return the membership card for a dependent (including the Elderly Recipient Certificate) which is no longer valid after deletion.
I got married Claim for Marriage Allowance  Remittance takes one month
Report the change of name Membership card under the new name shall be issued in two weeks.
Change the registered seal for Installment Savings Account Seal change request is accepted at all times.
Change the account name of Accumulated Fund Mutual Aid Annuity  When accepted on or before the 25th of the month, the change is reflected in the transfer of the month after the following month. 
Change the account name of Mutual Aid Term Insurance. When accepted on or before January 25 or July 25, the change is reflected in the transfer date of March 22 or September 22, respectively.  
Report the transfer of the Voluntary Continued Insurance Member. A new membership card shall be issued in two weeks.
Request for Approval of a Dependent Issuance takes two weeks.
Report category change or confirmation of National Pension Report to PMAC as to category change or confirmation of Category III National Pension Insured person.
A baby was born Claim Maternity Benefit/Dependent Maternity Benefit Remittance takes one month.
Request the issuance of Membership Card for the baby. The baby’s Membership Card shall be issued in 2 weeks
Claim Special Exception for Monthly Standard Salary When Caring for Children under Age 3 Issuance of the confirmation notice takes two weeks.
I got paid less due to maternity leave Claim Maternity Allowance. Remittance takes one month
I took maternity or childcare leave Apply for the premium exemption during the leave period. Apply upon leave.
I got paid less after my maternity or childcare leave Request revision in the standard monthly salary after leave. Monthly Standard Salary shall be revised after three months of return to work
I got paid less due to my absence through illness or injury Claim Sickness and Injury Allowance. Remittance takes one month.
I got hospitalized Claim hospitalization benefits covered by the Mutual Aid Term Insurance (Medical Insurance plan and Medical Expense Support plan)  Remittance takes two weeks.
I paid full amount of medical treatment expenses Claim reimbursement Reimbursement takes one month.
I had medical treatment overseas Claim reimbursement Reimbursement takes three months.
I got injured by a third party (e.g. in a traffic accident) and had health insurance treatment Contact PMAC for liability claim. We will immediately send you a set of documents for the claim.
I used a handicap transportation service to go to hospital or move to another hospital Claim the transportation expense benefit. Reimbursement takes one month.
I had a full medical check-up Claim medical check-up subsidy. Remittance usually takes one month, but it may take two months during the busy season.
I became disabled Claim the Disability Employees’ Pension  The pension amount shall be determined in two months.
You should apply to the implementing institution which you were with when the first medical examination was made.
I became seriously disabled Claim disability insurance benefits covered by the Mutual Aid Term Insurance (Survivor’s Annuity Insurance plan, Three Major Disease Coverage Insurance plan) Determination on the lump-sum amount or the pension amount takes two weeks. (The claim shall be reviewed based on the designated checkup list prior to the determination.)
A member of the group credit life insurance policy shall submit a report on the incident to be covered by insurance. Submit the Group Credit Life Insurance Application Form with a medical certificate that states the disability in a specified format.
I became eligible for the Old-age Employees’ Pension Claim the Old-age Employees’ Pension The pension amount shall be determined in two months.

I have turned 65 years Claim the Old-age Employees’ Pension (Regular Payment)  The pension amount shall be determined in two months.
Claim the Old-age Basic Pension Note that implementing institution differs depending on your membership history . Refer to Old-age Basic Pension
Claim the benefits upon complete the savings for Accumulated Fund Mutual Aid Annuity
  • The savings will be completed when you reach age 65, even if you are still at work then.
  • The insurance company will directly send a set of documents for the claim procedure to the member’s registered address two months and half before your 65th birth month.
I have turned 75 years When a dependent’s address differs from the member’s address, the dependent’s address should be reported to the Medical System for the Old Elderly. The dependent’s address notification should be submitted to PMAC.
Request cancellation of the Installment Savings Account
  • The cancellation procedure is required when you reach 75 years, as you will then become a specified personnel member.
  • Submit the “Installment Savings Withdrawal/Cancellation Form” to PMAC by the 25th of the month of the day which is one day before your 75th birthday so that remittance is made to your school account on the 20th of the following month.
I have retired Submit a disqualification notice Make sure to return the Membership Card and the Membership Card for a dependent (including the Elderly Recipient Certificate) which are no longer valid after retirement.
Apply for the Voluntary Continued Insurance Membership
  • Apply within 20 days of retirement
  • Issuance of the Membership Card takes two weeks
Become a family member’s dependent Follow the procedures of the family member’s medical insurance policy.

Request category change of National Pension. Consult with a national pension advisor in your municipality.
Request determination or revision of the pension amount. The pension amount shall be determined in two months
Repayment of the loan balance.
  1. Voluntary repayment of loan balance before retirement:
    If you apply on or before the 15th of the month, the Loan Repayment Advice (for voluntary/immediate repayment) will be sent within the same month. The loan balance shall be repaid according to the advice.
  2. Repay the whole balance upon retirement:
    The Loan Repayment Advice (for voluntary/ immediate repayment) will be sent upon confirmation of disqualification. The balance shall be repaid according to the advice. (An amount equal to the repayment balance must be deposited in your school beforehand.)
  3. The “Loan Completion Certificate” will be issued upon confirmation of your remittance of the balance.
Request the cancellation of Installment Savings Account
  • When the “Installment Savings Withdrawal/Cancellation Form” reaches us on or before the 25th of the month, remittance will be made to your school account on the 20th of the following month. 
  • Submit the “Non-Taxable Savings Discontinuation Form” when "Maruyu" (tax-exempt savings) has been applied.
Apply for withdrawal from the Accumulated Fund Mutual Aid Annuity, and claim benefits (by choosing the type of benefit).
  • If you apply for withdrawal, and claim benefits on or before the 25th of the month, you may withdraw from the pension scheme as of the end of the following month and the lump-sum remittance shall be made on the same day. When you have opted for the pension scheme, the pension shall be paid on the 20th of February, May, August and November.
  • To save up lump-sum premiums at retirement, the notification is due on the 25th of the month, two months before the month of retirement.
  • Voluntary Continued Insurance Members may maintain their membership.
Apply for withdrawal from the Mutual Aid Term Insurance (Personal membership plan) due to retirement.
  • When you plan to retire on or before the end of March or the end of September and withdraw from the insurance scheme, submit the withdrawal notice before the start of insurance coverage.
  • Voluntary Continued Insurance Members may maintain their membership.
A member passed away Submit the disqualification notice (this is also applicable to Voluntary Continued Insurance Members’ death) Return the Membership Card and Membership Card for a dependent (including the Elderly Recipient Certificate) which are no longer valid.
Claim the burial expenses Reimbursement takes one month
Claim Survivors’ Employees’ Pension. The pension amount shall be determined in two months.
Report category change of National Pension (for dependent spouse of less than 60 years of age.) Consult with a national pension advisor in your district.
Members of the Group Credit Life Insurance system with a loan plan must contact PMAC and submit an insurance accident report. After contact, a Group Credit Life Insurance application form and related documents will be sent to you. Please submit the form and documents along with the attached documents.
Apply for withdrawal from the Accumulated Fund Mutual Aid Annuity, and claim Survivorís Lump-sum payment or Pension payment for survivors. The request is accepted at all times. The date of the memberís death is the date of the memberís withdrawal from the Annuity. The Lump-sum payment shall be remitted upon completion of necessary procedures. (the pension is paid on the 20th of February, May, August and November)
Claim the death benefit of Mutual Aid Term Insurance (Survivor’s Annuity Insurance, Medical Expense Insurance, Three Major Disease Coverage Insurance).  In case of lump-sum payment, remittance takes two weeks. In case of pension payment, the pension amount shall be determined in 2 weeks. (The pension shall be paid on the 15th of February, May, August and November)
A dependent passed away Apply for cancellation of the dependent.
  • Issuance of confirmation notice on the cancellation takes two weeks.
  • Make sure to return the Membership Card for the dependent (including the Elderly Recipient Certificate) which is no longer valid.
Claim the Dependent Burial Benefit Payment takes one month.
Report the death to the National Pension Fund. Send the Category III National Pension Insured person’s death report to PMAC.
Claim the death benefit of Mutual Aid Term Insurance (Survivor’s Annuity Insurance, Medical Expense Insurance, Three Major Disease Coverage Insurance ). Remittance of the lump-sum amount takes two weeks.
My properties were damaged due to a disaster Claim Disaster Relief benefit and Supplementary Disaster Relief benefit.  Remittance takes one month.
The member or his/her dependent passed away due to a disaster Claim Condolence benefit or Dependent Condolence benefit. Remittance takes one month
I need a Certificate Request issuance of a qualification certificate (for the procedures of subscription to National Pension System or other health insurance schemes) Issuance takes one week; however, the certificate will be reissued on the spot if you take the procedure at our headquarters or at Mutual Aid Administration Division of Garden Palace.
Request issuance of certificates related to the payment of Short-term benefits. Certificates related to the payment of Short-term benefits shall be issued after confirmation of payment.
Request issuance of Deposit Balance Statement. Issuance takes one week
I need financial support Apply for a loan (e.g. general purposes, education, marriage, home, disaster, and medical treatment) When your application is accepted between the16th of the previous month and the 15th of the current month, the loan will be remitted to your school account on the 2nd of the following month.
*When your application is accepted between 16th and the end of the current month, the loan could be remitted to your school account on 22th of the following month if you so request.
