3. A copy of the portion of family register relating to the individual concerned can be replaced with a resident card. However, a copy of the portion of family register relating to the individual concerned is needed when the original family name or family relationship needs to be verified. Persons with foreign citizenship should attach a certificate of matters in the alien registration records.
5. Individuals who have been enrolled in a public pension system other than the Private School Mutual Aid System.
6. When the member receives the disability benefit or survivor's benefit.
7. When the member accepts the decision concerning public pensions other than the Private School Mutual Aid.
8. In the case of an individual who is or has been a member of the Employment Insurance Scheme
9. When the member applies for a job.
10. When the member applies for the old workers' salary supplement.
11. When an individual is enrolled in public pension system other than the Private School Mutual Aid, or when an individual became an assembly maker.

In the case of individuals with more than 20 years of enrollment in the Private
School Mutual Aid System the form will be sent automatically by the Private School
Association. |

1. Claim form for LumpSum Withdrawal Payment (including Confirmation of Pension System Membership History on Applying for the Lump-sum Withdrawal Payment)
